You will download your LAC credential application from the Licensed Addiction Counselor Program section of the BSD. Besides your personal information you will need:

• Official transcripts from all coursework
• Weekly time sheets from your clinical supervisor
• Fingerprint and background check

Although you will send your official transcripts you will also be required to list the individual coursework you’ve taken to show you have met the Board requirements for education.
